How To Get Started?
A Medicare EPC/CDM Program is created by your GP to help you manage a chronic medical condition. These programs can consist of visits to several different health providers, including Exercise Physiologists. If your GP identifies that you need to work with an EP, they will provide you with a referral to come see us.
We will work together with you, your GP, and your Allied Health team to create a treatment plan tailored to your needs. We will also send a management plan and reports to the referring GP along the way.

Frequently Asked Questions
Yes you can – however there are a few conditions.
To meet the criteria:
- You must have a chronic condition that you have suffered with for more than 3 months
- Your GP must refer you to a specific clinic for EP services i.e. Assure Rehab
- Your GP must complete a Team Care Arrangement or GP management plan to go with your referral
Once we have received the referral that is addressed to our clinic and a management plan from your GP, we’ll be able to book you in under Medicare services.
The Medicare EPC Program allows up to 5 rebated consults per year to any referred allied health professional. This includes several different services, including Exercise Physiology.
Your GP may refer you to one service for all 5 sessions, or they may split these sessions between multiple providers.
For Example: Your GP may refer you for 5 EP sessions. Or they may refer you for 2 Podiatry consults and 3 EP consults. Or 1 Dietetics consult, 2 Physiotherapy consults, and 2 EP consults. Any of these scenarios would make up your total limit of 5 rebated consults per year. The possibilities are endless!
This number is renewed each new year, but you will also need a new referral each year.
If you are of Aboriginal or Torres Strait Island descent, please speak to your GP in regards to additional Medicare rebates available to you.
The current Medicare rebate will cover 100% of your EP sessions, for up to five visits per year.
There is no out-of-pocket expense for you on these sessions.
There is no list of eligible conditions.
It is up to the GP to plan and coordinate the care for patients with complex or chronic conditions.
Every session is different, but we will always start with a mental health check-in. We believe mental health is a strong component to your overall wellness – this is why we focus both on your mental and physical health prior to starting your program for the session. We take into account all aspects of your health to ensure we prescribe the safest and most effective programs possible.
If required, we may do measurements such as blood pressure or pain levels. These will be completed both pre- and post-exercise. All sessions will commence with a warm-up, whether it is cardiovascular exercise, dynamic exercises, or mobility work to increase muscle activation and blood flow. Warming up will reduce your risk of injury.
A structured program will be prescribed, consisting of various exercises aimed at achieving your goals. Each session will be different as we try to prescribe a variety of exercises that are fun and challenging. Your session will finish with stretching to reduce muscle soreness post-session.
Yes! We accept a wide variety of private health funds for EP. You will need to contact your private health fund to determine your coverage.
Anytime! You can see an EP even if you are healthy and want personal training sessions.
We see an extensive range of people including people who are going through cancer treatment, recovering from injuries, pre -and post-surgery clients, and people who want to improve their level of independence at home.
Depending on your reason for attending, you can see results as early as the first session. However, we typically see results in 2 to 6 weeks.

We recommend seeing your EP a minimum of once per week. However, 2 or 3 sessions per week is ideal to get maximum results from your treatment.
EPs are trained extensively in the management of injuries and chronic diseases. They have a four-year university degree, specialising in human anatomy and biomechanics.
Even though we specialise in medium- to high-risk clientele, we are qualified to assist everyone with their exercise journeys. As an EP, we can tailor programs to suit the individual patient’s needs, not just generic programs.
For your exercise session:
- Wear closed-in athletic shoes and socks
- Bring a water bottle and sports towel if you desire
- Wear loose, comfortable clothing that allows you to move freely as you may be asked to perform a range of movements
- Proof of Covid-19 vaccination and photo ID (This is a WA mandated requirement)
